HEALTH RISK ASSESSMENT
A health risk assessment (HRA) is a health questionnaire used to provide individuals with an evaluation of their health risks and quality of life. After employees complete the HRA they receive a wellness report with values and recommendations to improve their health. The company receives an aggregate report with areas for improvement and recommendations to improve the health of their employee population.
ON-SITE SCREENING TEAM
A clinical on-site screening team will be provided for companies preferring to have employee screenings inside their facility. Assessment includes:
- Test for glucose, total cholesterol, HDL, LDL & triglycerides
- Biometrics (height, weight, waist measurement, BMI, blood pressure and pulse)
- Sleep disorder questionnaire
The biometrics measurement will provide crucial measurements of blood pressure, pulse, height, weight, BMI and waist measurement. This data is entered into our online Health Risk Assessment that provides a comprehensive look at the personal health of each individual. These health risk assessments will provide a complete aggregate report on the full population for management. In addition, each participant may view their personal health and wellness profile and score online and share this information with their primary care physician.
